We sometimes get misunderstood through social media at #FunctionalRangeSystems. PROM is incredibly important. It is the baseline for movement and the prerequisite for mobility. Which is why it’s one of the first and most important qualities we look at in the #FunctionalRangeAssesment. You need degrees of freedom of motion before you can learn how to USE those ROM’s.

When a person walks into a clinic with suspected “plantar fasciopathy”, it's a good idea to spend ~5 mins to clear the spine with repeated movements; that is before proceeding to the peripheral joints. For more info on repeated movements, click here #McKenzieTherapySeries.

Why clear the spine before peripheral joints? Well…this is because of lumbar somatic referred pain. It can mimic conventional orthopedic conditions (i.e., groin, meniscal, heel pain) and is more common than you think. Somatic structures are: ligaments, facets, joint capsule, vertebral bodies, dura mater, muscles, intervertebral discs; thus, somatic referred pain indicates referred pain caused by such somatic structure(s). Signs & symptoms are: likely present with direction preference (#McKenzieTherapySeries for more info), poorly localized, deep, aching, diffuse, dull, heavy, cramping, or gripping sensations.

Proposed mechanism behind somatic referred pain is that multiple afferent neurons from different areas of the body converge onto a common interneuron before being relayed to the brain 🧠. Once the impulse reaches the brain 🧠, the original source of pain cannot be differentiated 🤷. This is why some may experience lower extremity (LE) pain that has lumbar spine (L/S) origin.

If we suspect LE pain (i.e., heel pain) that has L/S origin compared to “true” plantar fasciopathy, treatments can look different such that with somatic referred pain, it may be worthwhile to direct the treatment to the L/S compared to the foot. The caveat is, when the L/S is suspected as a source of pain, most clinicians will overemphasize nerve root, segmental level and the dermatomal map to guide their clinical decisions.

However there is no ❌ conclusive evidence to suggest somatic referred pain is caused by hypersensitivity of the corresponding segmental level. For example, if a person presents with lumbar somatic referred pain in the right heel, and you decide to use manual therapy, you do not have to isolate and specifically target the right S1-2 nerve roots. You can virtually press anywhere in the L/S and get the desired response.

2️⃣ Para cada país, pensaban preguntas tipo concurso (what is the longest river in...?) y buscaban información para preparar 4 respuestas.
Just comment below! ⤵️⤵️.

Today, I evaluated a kindergarten Spanish speaking child and he completely made my day. 👩🏽‍🏫
He was polite, caring and very knowledgeable. He scored within the average range 🤓 👏🏽 But this is not what I will remember. What I will remember is that every second he had, he wanted to talk about his “hermanito” (younger brother). He spoke so proudly of him. His eyes glazed with happiness and he couldn’t stop smiling. At the end of testing, I offered him a sticker and he said “ I will give this to my hermanito.” Melted my ♥️ Kuddos to his parents for doing a phenomenal job in parenting👨‍👩‍👧‍👦 #speechlanguagepathology
